If Pulse is very feeble - ? Left ventricular failure. If Liver is palpable, tender with Leg Edema - ? C.C.F. If only lung signs-Primary Lung disease If L.V.P
1. Oxygen 2. Inj. Morphin 15mg IM or Fortwin/Norphin (3B-4 to 7) 3. Inj. BETNESOL 2-4 amps IV or Decardron/Hydrocortisone (9A-2,3 & 5) 4. Inj. LASIX 1-2 amps I.V.V (Frusemide = 6 B-1) 5. Inj. Aminopyllin 10ml + 25% dextrose 10ml x slow IV. (5D-2) 6. Refer to Hospital to rule out Myocardial infarct.
If C.C.F.,
1. Propped up position. 2. Oxygen. 3. Inj. Aminophyllin 10ml + 25% Dextrose 10ml x slow I.V. (5D-2) or Inj. DERIPHYLLIN 2cc IM (5D-7) 4. Inj. LASIX 2 amps I.V. (Frusemide =6B-1) Inject IM if BP is low. 5. Tab LANOXIN 3 stat, then 1 tds x 3 days, then 1 OD 6. Tab CALMPOSE 5mg 1 stat. 7. No IV fluids, No saline (No salt.)
If Lung Disease,
1. Oxygen 2. Inj. Aminophyllin 10ml + 25% Dextrose 10m x slow I.V (5D-2) or Inj. DERIPHYLLIN 2 cc IM stat (5D-7) 3. Inj Decardron 2 cc I.V. stat (9A-2,3 &5)
Get X-ray Chest as early as possible to decide further line of treatment.
